| *Woodhaven Swim and Tennis Club has reached full membership for the 2011 summer season. We are accepting reservations for the waiting list for the 2012 season. The Woodhaven Board of Trustees issues memberships on a first come, first serve basis and does not discriminate for any reason. If you would like to be placed on the Woodhaven Swim Club Waiting List for 2012/2013, please send an email (be sure to include your name, address, home phone, mobile phone and email address) to: You will receive confirmation of your place on the waiting list within one week of sending your email. Please Note: Due to a technical error, if you requested to be placed on the 2012 waiting list between May 11th & July 8th 2011, please resend your request to the email above.* Initial membership requires three types of fees to be paid in the first two years and they are as follows: |

| Annual Fee: | Single Adult | $280.00 + 18.20 Tax | = Total $298.20 |
| Family with 1 child | $300.00 + 19.50 Tax | = Total $319.50 | |
| Family with 2 or more children | $320.00 + 20.80 Tax | = Total $340.80 |

The fees may be paid in two installments. The Initiation Fee and the Annual Fee are due when the top part of the application form is submitted. ALL PERSONS USING WOODHAVEN SWIM CLUB, INC.
